Peer Case Management

Peer Case Management is a peer approach of case management that utilizes the core activities of case management to provide a wide range of needs and services to assist people in the community. This promising practice utilizes peers who have lived and professional experience to provide assistance and support in accessing medical, behavioral health, addictions, housing, employment, and educational services. They act as a point of contact between the people they serve and the resources needed to live fuller and healthier lives. The Behavioral Health Initiative has funded two contracts that were implemented in July 2019 to Centro Savila and Crossroads for Women aftercare program to provide Peer Case Management to individuals age 14 years and older with a primary diagnoses of mental illness.

Centro Savila ensures access to linguistically and culturally relevant, quality mental health and prevention services, education, and healthcare professional development to improve the mental health of the community. Centro Savila provides affordable, bilingual, prevention, and behavioral health treatment as well as Peer Case Management services to reach the difficult populations such those who are low income and/or uninsured adults who may experience one or several unmet needs, homelessness, disconnected from services, urban Native Americans, undocumented and/or limited English proficient immigrants, hunger, and those parenting young children.

Crossroads for Women provides comprehensive integrated services to empower woman emerging from incarceration to achieve safe, healthy, and fulfilling lives in the community, for themselves and their children. Programming includes transitional and permanent supportive housing and those who graduate from the housing programs have access to Peer-On-Peer Support Services (POPSS) Aftercare Services to help them achieve long term goals and utilize coping skills to maintain life recovery. Individualized support services are provided to women in recovery to support them through destabilizations and/or to maintain the stability.
